Actually, it is rare that an accident would be the cause for the necessity of rotator cuff surgery. Typically, the need for such is many years of a repetitive action. Painters are an example of a vocation wherein a goodly number of such require surgery of that type. It's the bone-on-bone that is the normative requisite for diagnosing such.

A fall, or similar accident, would rarely cause such a need.
